If you would like to coordinate or are missing from the list, please contact shayes@coloradocoalition.org. Coordinators may have region-specific forms and information that might look different from the forms below. The training offered on this webpage covers the BoS CoC Point-in-Time Surveys, which cannot be altered by region. However, support documentation may be altered to support regional needs.

- BoS CoC 2023 Point-in-Time (PIT) Sheltered Count Form. Sheltered Point-in-Time Survey for people experiencing Sheltered Homelessness in Emergency Shelter and Transitional Housing programs only. CANNOT be modified.
- BoS CoC 2023 Youth Supplemental Survey (YSS) Questions, Updated 1-5-2023. Youth Supplemental Survey to go along with the above “BoS CoC 2023 Point-in-Time (PIT) Sheltered County Form" for youth ages 11-24.
- BoS CoC 2023 Permanent Supportive Housing (PSH) Editable Survey Form (Count of Individuals in Households Only). Use this form for the required count (no additional survey is needed).
- BoS CoC 2023 Rapid Re-Housing (RRH) Editable Survey Form (Count of Individuals in Households Only). Use this form for the required count and breakdown of family type (no additional survey is needed).
